During the film, Joel opens up about his affair with a bandmate's wife, which led to two suicide attempts and a coma. The affair caused the breakup of his band and friendship, leaving Joel in a downward spiral of depression and self-destructive behavior.

Following the affair, Joel found himself homeless and struggling with intense emotional pain. He attempted suicide by overdosing on sleeping pills, which resulted in a coma. After waking up, he made a second suicide attempt by drinking "lemon Pledge," but was saved by his friend Jon Small, who rushed him to the hospital. Despite the strain on their friendship, Small ultimately forgave Joel for the affair, recognizing the depth of their bond.

After his suicide attempts, Joel sought help and checked himself into an observation ward. He emerged with a renewed perspective, channeling his emotions into his music. Unfortunately, Joel's ongoing health issues, including a diagnosis of normal pressure hydrocephalus (NPH), forced him to miss the premiere of his documentary and cancel upcoming performances. He is currently undergoing physical therapy and taking a break from performing to focus on his recovery.
