Originally planned as "Wanted Dead or Alive," the band posed in cowboy attire for the cover shoot, which they found unsatisfactory. However, fate led them to the final title and cover concept when their manager spotted a "Slippery When Wet" sign on the road. Inspired by a strip club performance, they improvised a cover using a Hefty trash bag with water sprayed on it, with Jon Bon Jovi writing the title on it. Despite the unconventional cover, the album's hit tracks like "Wanted Dead or Alive," "Livin' on a Prayer," "Never Say Goodbye," and "You Give Love a Bad Name" ensured its massive success.
